You are notified that an action has been commenced in the Circuit Court of Barton County, State of Missouri, at 1004 GULF ROOM 204, LAMAR, MO 64759, the object and general nature of which is the care and custody in and to the above-named juvenile under the age of eighteen years.

You are further notified, pursuant to Section 211.211 RSMo., of your right to have counsel, and if you request counsel and are financially unable to employ counsel, counsel shall be appointed by the Court.
